Pop Warner Football 

All football players are required to complete the following documents before rosters are locked on August 1, 2025.  Once our books are certified, any athlete without full and complete paperwork will be deemed ineligible to play.  

Need to be physically turned in at Equipment Handout:

• Certified birth certificate.  This must contain the embossed seal and can be obtained from the county in which your child was born. These copies will be returned to parents as soon as Book Certification is complete.  If you need to obtain a copy, please visit the Kane County Clerk.
• Photocopy of birth certificate.
• 2024-2025 report card containing the grades for the entire year.
  • If your child is home schooled, please complete the home school form found here.
  • If your child was in preschool for the 2024-2025 school year, please complete the preschool form found here.
• Sports Physical dated 1/1/2025 or after.   The IESA form can be found here.  We are excited to announce we will be once again offering on-site sport physicals on 6/7/25.
  
• Uniform Deposit check post dated 12/01/2025 for $450 (returned at the end of the season)
• Volunteer Deposit check post dated 12/01/2025 for $200 (returned at the end of the season if volunteer credits are completed)
